The Michigan stat is absolutley damning - 61.6% MA penetration with half the plans using prior auth, yet it ranks 5th worst in preventable hospitalizations. That's exactly the opposite of what managed care is supposed to achive. The whole 'private sector efficiency' argument falls apart when you look at the Commonwealth Fund data. UNH and the other big players have basically turned MA into a profit extraction machine rather than a quality improvement program. The star rating gaming you mentioned is also crazy - combining random counties to manipulate scores shows they know the system isn't delivering what they promise.
